100% PERMANENT Hair Removal & Skin Care is committed to providing a relaxing atmosphere. The following Policies are intended to enhance everyone’s experience with us.
Booking & Arrival Policy
All services require a credit card, gift certificate, to guarantee a reservation. You will not be billed unless there is a cancellation or no show with less than 24 hours notice.
Please arrive a minimum of 15 minutes in advance for your treatments. However, the length of service may be adjusted so as do not interrupt the scheduled appointments of other guests. Full price of scheduled service(s) will apply. Clients who arrive late will be charged in full for their scheduled session, but will receive an abbreviated, pro-rated session. For example, if you scheduled an hour session and arrive 10 minutes late, you will be charged for that hour session but will only receive 50 minutes of treatment for that session, including consultation and changing time. If you had a large volume electrolysis appointment (4 hours of electrolysis or more) and if you didn’t show up in the first 45 minutes of your treatment your appointment will be cancelled and you will be charged 100% of the scheduled service price. If you had a traditional electrolysis appointment and you didn’t show up in the first 15 minutes of your treatment your appointment will be cancelled and you will be charged 100% of the scheduled service price.
100% PERMANENT INC guests must be 18 years of age or older to receive treatments without the approval or accompaniment of an adult.
Cancellation Policy & Fees
Your electrolysis treatments are reserved especially for you. We value your business and ask that you respect our scheduling policies.
We understand that unanticipated events happen occasionally in everyone’s life. Business meetings, project deadlines, car problems, snowstorms, and illness are just a few reasons why one might consider canceling an appointment. In our desire to be effective and fair to all of our clients and out of consideration for our time we have adopted the following policies
Should you need to cancel or reschedule, please notify us at least 24 hours in advance.
Any cancellations with less than 24 hours of notice are subject to a cancellation fee of 50% of the full service price.
Any large volume electrolysis appointments or any appointments longer than 4 hours of electrolysis cancelled with less than 2 weeks of notice are subject to a cancellation fee of 50% of the full-service price.
Clients who miss their appointment(s) without any prior notification will be charged 100% of the scheduled service price. Missed consultation appointments without prior notification will be charged a $50 fee. We recognize the time of our clients is valuable and have implemented this policy for this reason. Please call us at (252) 722-5006 if you’re unable to keep your appointment.
As a courtesy, we will notify you to confirm your service appointments two business days prior to your appointment date. However, if we are unable to reach you please understand that it is your responsibility to remember your appointment dates and times to avoid late arrivals, missed appointments and the no show fee.
Pricing, Services, & Payment Methods
Prices and service availability are subject to change without notice. We will make every effort to inform our clients of any changes that may occur but encourage all clients to inquire about pricing and available services when booking.
100% PERMANENT INC Beauty Spa accepts Visa, Master Card, Discover, American Express, 100% PERMANENT gift cards, bank debit cards, cash and personal checks.
A $25 fee will be assessed for returned checks.
100% PERMANENT INC Beauty Spa gift cards are not redeemable if reported as lost or stolen.
Gratuity is not included in the price of service or spa packages. Customary tipping for spa services is between 15% to 20% of the total treatment cost; however, it is up to your discretion. You may tip the therapist in the room or upon checkout at the front desk with cash or check only.
All service sales and gift card sales are final.
Retail items can only be refunded if unused, in its original packaging, and presented with a receipt within 14 days of purchase. In some situations, a restocking fee will be charged.
Refunds will be issued in Store credit only.
At 100% PERMANENT INC Beauty Spa discretion, some non-receipted returns accompanied by a photo ID may qualify to receive a gift card for the lowest selling price. All returns are subject to system verification.
To monitor client progress we may take photographs of treated area .By signing this form, you give permission for your photo(s) to be kept on file and used for education or advertising. Privacy bars will be applied if photos are used.
Privacy & Safety
100% PERMANENT INC Beauty Spa upon checkout, guests may choose their method of payment. will not share your personal information except when necessary to be perform bussines function.Treatment records are kept confidential.Please inform upon booking if you prefer that we do not leave voicemail messages for you or if you have other contact instructions.
Clients with medical conditions are advised to consult their physicians prior to booking any electrolysis services. Kindly inform us of any existing medical conditions by completing a client information form at your first appointment. We reserve de right to discontinue a service for any reason to ensure the safety of all parties. Information provided by 100% PERMANENT INC Beauty Spa is not intended to be a substitute for professional medical advice, diagnosis, or treatment and clients with medical questions should seek the assistance of a licensed physician.
Waiver & Release of Liability
When necessary, I hereby authorize 100% PERMANENT Beauty Spa ownership/stuff to apply client-supplied topical numbing product to the specified treatment area and release 100% PERMANENT INC Beauty Spa ownership/stuff of all liability for any complications that may arise as a result of the use of numbing agents. Furthermore, I hereby release 100% PERMANENT INC ownership of all liability for any complications ,injury, harm, and/or abrasion(s) that may arise as a result of receiving services ai 100% PERMANENT INC at 2301 N Clark Street ,Suite 1,Chicago IL 60614
