Booking & Arrival Policy
- All services require a credit card or gift certificate to guarantee a reservation.
- A 50% deposit is required to reserve an appointment. This deposit will be credited toward your service fees on the day of your appointment. If you fail to cancel or reschedule within 24 hours of your appointment, your deposit will be forfeited.
- If you cancel your appointment through your Vagaro account at least 24 hours in advance, the full amount (less any service fee) will be refunded immediately. Refund processing times may vary depending on your bank or credit card provider. Cancellations made via text or phone call may take 24-48 hours to process.
- Emergency absences will be reviewed on a case-by-case basis.
- Please arrive at least 15 minutes before your scheduled treatment. If you arrive late, your session may be shortened to avoid disrupting other scheduled appointments. The full price of the service will still apply.
- Clients arriving significantly late will have their appointments canceled and will be charged 100% of the scheduled service price:
- Electrolysis (4+ hours): Appointment is canceled if the client does not arrive within 45 minutes.
- Traditional Electrolysis: Appointment is canceled if the client does not arrive within 15 minutes.
- Clients must be 18 years or older to receive treatments without parental approval or accompaniment.
Cancellation Policy & Fees
At Electrolysis 100% Permanent, your appointments are reserved exclusively for you. We value your time and appreciate your respect for ours. We understand that unexpected events—such as illness, emergencies, or scheduling conflicts—can arise. To ensure fairness and efficiency for all clients, we have established the following cancellation policy:
Cancellation Notice Requirements:

Appointments 1 hour or less: Minimum 24 hours’ notice

Appointments longer than 1 hour: Minimum 72 hours’ notice

Appointments of 4 hours or more: Minimum 14 days’ notice
Failure to provide adequate notice will result in a charge of 100% of the scheduled service fee.
Providing timely notice allows us to offer your appointment slot to another client and keeps our schedule running smoothly.

Appointment Reminders:
As a courtesy, we send email and text reminders before your scheduled appointment. However, it remains your responsibility to remember your appointment time. Missed appointments, late arrivals, and no-shows may result in applicable fees.

Pricing, Services, & Payment Methods
- Prices and service availability are subject to change without notice. We encourage clients to confirm pricing and service availability when booking.
- We accept Visa, MasterCard, Discover, American Express, 100% PERMANENT gift cards, bank debit cards, cash, and personal checks.
- A $25 fee applies to returned checks.
- Lost or stolen 100% PERMANENT gift cards are non-redeemable.
Gratuities
- Gratuities are not included in the price of services or packages.
- Customary tipping is between 15% and 20% of the total treatment cost, but it is at your discretion.
- Tips may be given directly to the therapist or at the front desk (cash or check only).
Return/Refund Policy
- All service and gift card sales are final.
- Retail items may be returned for store credit if unused, in original packaging, and accompanied by a receipt within 14 days of purchase. A restocking fee may apply.
- At our discretion, returns without a receipt may qualify for a gift card at the lowest selling price, provided a valid photo ID is presented.
Photo Release
- To monitor treatment progress, we may take photographs of the treated area. By signing this form, you grant permission for your photos to be kept on file and potentially used for educational or advertising purposes. If used for advertising, privacy bars will be applied for anonymity.
Privacy & Safety
- 100% PERMANENT INC. will not share your personal information except when necessary for business operations.
- Treatment records are kept confidential.
- Please inform us if you prefer that we do not leave voicemail messages or if you have specific contact preferences.
- Clients with medical conditions should consult their physician before booking electrolysis services. A client information form must be completed at the first appointment.
- We reserve the right to discontinue a service at any time to ensure the safety of all parties.
- Our information is not a substitute for professional medical advice. Clients with medical concerns should consult a licensed physician.
Waiver & Release of Liability
- When necessary, clients may authorize 100% PERMANENT INC. staff to apply client-supplied topical numbing products. By doing so, clients release the spa and its staff from liability for any complications arising from their use.
- Clients also release 100% PERMANENT INC. from liability for any injuries, complications, or harm resulting from services received at our facility: 1315 N Larrabee St, Chicago IL 60610.
